Patient Information
Neurological tests done or ordered by Drs. Rosenkilde and Caporaso include:
Blood tests, CT or MRI of brain and spine, PET imaging, carotid and transcranial doppler ultrasound (blood flow to and inside the brain), neuropsychological or cognitive testing, lumbar puncture (spinal tap), EEG (electroencephalography), electromyography and nerve conduction studies (EMG/NCV), evoked potentials.

What do I need to bring to my first visit?
- Insurance information
- Referral from your primary physician if you belong to an HMO
- List of your current medications
- Relevant medical records (particularly any recent neurological test results)
- Actual MRI or CT films with reports, if consult concerns such studies
- Active case number for W.C. conditions.
If my condition is related to a work injury or a motor vehicle accident, what specific information will I need?

How can I help the first visit go smoothly?
- You will be asked to fill out several forms while in the Waiting Room. Therefore, be sure to come on time.
- While in our office, we kindly request that you avoid wearing perfumes, lotions or aftershaves as other patients may have severe reactions to these.
- During the consultation, one family member, or friend, may accompany the patient. (Presence of multiple family members can have negative impact.)
- Please turn off cellular phones and beepers while in the office.
